When Routine Becomes Risk: The Hidden Danger of Familiarity in the Workplace

A recent chemistry classroom accident in Ohio has drawn national attention after a 15-year-old student suffered life-threatening burns during a laboratory experiment. The investigation remains ongoing, and at this point, the exact sequence of events that led to the incident has not been publicly released.
It is important to recognize what we do not know.
We do not know whether equipment failed. We do not know whether an unforeseen chemical reaction occurred. We do not know whether every procedure was followed exactly as written or whether an unexpected series of events unfolded despite appropriate precautions. Until investigators complete their work, any attempt to assign blame or speculate about the cause serves no one.
What we can do, however, is reflect on one of the most persistent challenges facing every organization that values safety.
The greatest threat to a mature safety culture is often not a lack of knowledge. It is familiarity.
Whenever employees perform a task for the first time, they tend to approach it with a heightened sense of awareness. Instructions are read carefully, equipment is inspected thoroughly, and each step is completed with deliberate attention because the process is still unfamiliar.
Over time, however, repetition changes our perception of risk.
As a task becomes routine, the human mind naturally begins to conserve mental effort. Actions that once required conscious thought gradually become automatic. This efficiency is one of the remarkable strengths of human behavior, allowing experienced employees to work more confidently and productively. Yet the very process that makes us more efficient can also make us less attentive to the hazards that have always existed.
This transition is rarely intentional.
No one arrives at work planning to ignore a safety procedure. Few employees consciously decide that personal protective equipment is no longer necessary or that a shortcut is worth the additional risk. Instead, complacency develops gradually, almost imperceptibly, as repeated success reinforces the belief that nothing will go wrong because nothing has gone wrong before.
That is precisely what makes complacency so dangerous.
Most shortcuts do not result in immediate consequences. Most overlooked hazards do not immediately produce injuries. In fact, people often become more confident after getting away with a shortcut several times, reinforcing behaviors that slowly drift away from established safe work practices.
Then, one day, the conditions change.
A variable that was insignificant yesterday suddenly matters. A small oversight combines with an equipment issue, an environmental condition, or an unforeseen circumstance, and what had become a routine task is transformed into a serious incident.
Again, this is not a statement about what occurred in the Ohio classroom. The investigation will determine the facts, and those facts deserve to be respected. Rather, the incident serves as a reminder of a universal principle that applies across every industry: routine should never be mistaken for the absence of risk.
In many respects, the setting itself makes this event especially thought-provoking.
High school chemistry laboratories are designed with education and safety in mind. Experiments are typically selected because they are considered appropriate for students, hazards are evaluated before activities are introduced into the curriculum, instructors receive training, and safety equipment is expected to be readily available. While no environment is completely without risk, educational laboratories generally operate under carefully controlled conditions intended to minimize the likelihood of serious injury.
Now consider the environments in which many employees work every day.
Manufacturing facilities operate powerful machinery capable of causing catastrophic injuries within seconds. Warehouses rely on forklifts, conveyors, and material handling systems that demand constant awareness. Chemical processing facilities manage hazardous substances under pressure and temperature conditions that require strict operational discipline. Laboratories handle corrosives, flammables, oxidizers, toxic materials, and reactive compounds. Maintenance personnel perform electrical work, enter confined spaces, and complete repairs at elevated heights. Transportation workers operate commercial vehicles in unpredictable conditions, while healthcare professionals routinely encounter biological hazards that cannot be eliminated entirely.
Even organizations with exemplary safety programs, comprehensive training, well-written procedures, and full compliance with OSHA regulations cannot eliminate every hazard inherent in these work environments.
Safety has never been about eliminating all risk.
It has always been about recognizing risk, managing it effectively, and maintaining disciplined work practices every single day.
That is why safety programs must extend far beyond written policies and annual training sessions. They must actively guard against the gradual normalization of risk. Leaders must consistently reinforce expectations, supervisors must recognize the early signs of complacency, and employees must understand that experience should strengthen their respect for hazards rather than diminish it.
One of the most valuable questions any organization can ask is remarkably simple:
"Have we become comfortable simply because nothing has happened yet?"
History demonstrates that many serious workplace incidents occur during tasks that employees have completed successfully hundreds or even thousands of times before. The absence of previous injuries does not prove that a task is safe. More often, it indicates that the existing controls have worked or that circumstances have simply been favorable. Neither should be interpreted as permission to relax established safety practices.
Every routine task deserves the same level of preparation, attention, and respect as it did on the day an employee first learned how to perform it.
Our thoughts remain with the injured student, her family, her classmates, and the educators whose lives have also been affected by this tragedy. While we await the findings of the investigation, there is already one lesson that every employer can take from this event.
Routine should never lead to complacency, and familiarity should never replace vigilance.
The safest organizations are not those that assume routine work is inherently safe. They are the organizations that understand routine work is precisely where complacency is most likely to develop and where continued attention to safety is needed the most.
